Vocal transition is not just a matter of “pitch” (low or high). It is a complex process encompassing timbre, resonance, articulation, and even vocal gestures. Depending on the pathway (MtF or FtM), the biological and technical challenges differ considerably.

FTM Transition: The Effect of Male Hormones

For transmasculine individuals (FtM), testosterone hormone therapy has a direct and irreversible effect on the vocal organ. Testosterone causes thickening of the vocal folds, which naturally lowers the fundamental frequency (the voice becomes deeper).

However, lowering pitch alone is not always sufficient to create a “voice perceived as masculine.” Many FtM individuals experience:

  • Vocal fry: Fatigue related to the muscles adapting to the increased mass of the vocal folds.
  • Lack of chest resonance: The voice remains in the throat without using the body’s power.
  • Passaggio instability: Similar to a delayed adolescent voice change.

MTF Transition: Increased Structural Complexity

For transfeminine individuals (MtF), the challenge is structurally more complex. Unlike testosterone, estrogens have no effect on the size of the vocal folds or the larynx after puberty. Once the male puberty voice change has occurred, the structure remains fixed.

Studies (notably Davies, S., & Johnston, S.) highlight that vocal feminization relies on complete retraining of laryngeal function. It is not about “speaking in head voice” (which is tiring and unnatural), but about modifying the resonance of the vocal tract.

Why is this complex?

  • The resonance space must be reduced (raising the larynx in a healthy manner) without creating muscular tension.
  • Work must be done on brighter vowels and more precise articulation.
  • The risk of muscle tension dysphonia is high if the person tries to “raise” the voice solely through muscular effort.

Shortage of Speech-Language Pathologists and Rise of Vocal Coaching

Currently, many individuals in transition face a difficult reality: a shortage of speech-language pathologists specialized in gender transition. Waiting lists extend over several months, even years, which can exacerbate identity-related distress.
